Admission Criteria

 

Children enrolled in Connections and Tapestry are in need of treatment that is more intensive than traditional outpatient services and/or require multiple sources of support including intensive mental health community support, coordinated care and case management to live successfully in the community.

 

PEP Connections is designed to serve children and youth who:

 

·        Have been diagnosed with a serious emotional disturbance;

 

·        Are involved with two or more of Cuyahoga County's child-serving systems such as the Department of Children and Family Services, Juvenile Court, Board of Mental Retardation and Developmental Disabilities; and

 

·        Are either at serious risk of removal from their families and the community, or are returning to their families and the community from placement.



PEP Tapestry serves youth who:

·        Have been diagnosed with a serious emotional disturbance that significantly interferes with their ability to learn, work, and enjoy their lives;

·        Live in Cleveland, East Cleveland or Euclid; and

 

·        Are involved or at risk of involvement with Cuyahoga County's child-serving systems such as Department of Children and Family Services, Juvenile Court, or Special Education.
